University of Phoenix Student Teaching Evaluation & Documentation Process (Faculty Supervisor User Guide)

During student teaching the university will be utilizing a system called TK20 and its “Field Experience” binder application to support the collection of data and assessments necessary to be documented during the experience. The binder serves as a portfolio of work where the student, cooperating teacher, and Faculty Supervisor will all be able to access and document necessary requirements of their role throughout student teaching. The following instructions will provide guidance for Faculty Supervisors and how they will be utilizing the binder to submit their documentation and complete student teaching evaluations.

Accessing the System

To access the Field Experience Module Binder, click on the Assessment Management Tool link, listed under the Quick Links menu or bottom of the page of your faculty portal.

After clicking on “Assessment Management Tool” you will then be launched into the TK20 system. Once you are in, navigate to the “Field Experience” Tab located on the left side panel

After clicking on “Field Experience” you will click on the sub tab that generates “Assessments”. From there a list of students assigned to you will be visible.

To access the student, individually click on the hyperlinked name of the student to enter that binder.

Note: The red flag located next to the student’s name indicates that there is work to be done in the binder. The red flag will only disappear once all forms have been completed and the binder is submitted at the end of your supervising experience with the student.

Working inside the student’s Binder

Once you have clicked on the name the binder will open into a two-panel view.

Any work submitted by the student will be located on the left-hand side of the two panel. Anything completed by the Cooperating Teacher will be located on the right at the bottom of the page under “Cooperating Teacher’s Forms”. Your specific forms as a Faculty Supervisor will be located on the right side as well under “Field Experience Forms”. The red flag located next to each form indicates that this form needs to be completed.

Forms to be completed by the Faculty Supervisor

The forms that are assigned in your section of the binder will vary by program and documentation requirements. Below will outline how to access and navigate the various types of forms that are applicable within the binder.

FS Orientation Checklist Acknowledgement & Notes

The first form you will see under the faculty supervisor forms is a form titled “FS Orientation Checklist Acknowledgement & Notes”.

In this form you will be asked to type your name in the provided field to acknowledge that the orientation checklist was reviewed between the student teacher, cooperating teacher, and faculty supervisor. Additionally provided is a file upload section in which you can use to upload any notes or additional documents that you may need to as well as there is a site contact history section.

Site Contact History

In this section you can use to document contact efforts with the students cooperating teacher and any notes regarding that contact that would be important to document. To access click on the green + button to trigger the contact table. Select date of contact and use the single line text for notes.

Once you have typed your name, click “Save & Close” to save entry into the binder.

Student Teaching Evaluation Forms

As a faculty supervisor you will be tasked with completing the student teaching evaluation at midterm and final. Within the binder there will be two evaluations listed that will be specifically labeled midterm and final respectively. To complete the evaluations simply click on the name of the form to open the evaluation and subsequent questions. The evaluation will open on the right-side of the two-panel page. Click on the appropriate ratings throughout the evaluation form as well as checking whether the student is meeting requirements at the bottom of the form.

NOTE: Evaluations for student teachers may vary by program and examples reflected below may appear different for some.

When you have finished completing the evaluation click on the “Save & Close” button at the bottom of the page to save your responses.

Note: Each form will have a “Grade” text space throughout the binder. Please disregard this space. You will not enter anything into the “Grade” section. Please defer to the “Grade Form” also listed in the binder to score appropriately.

Informal Student Teacher Observation

For Non-California programs there will be 2 informal student teacher observation evaluations located in your forms. The Informal Observation is conducted during week three of the placement.

You will simply need to click on the name of the form to open. Complete each criteria section as requested and utilize the “Save & Close” button to capture your inputted responses.

CA Only-Pencil and Paper Evaluations

For CA students only, Faculty supervisor’s will complete the paper evaluations now inside the students binder along with the midterm/final evaluations. To complete the evaluation simply click into the form and complete as you would the midterm and final. When you have finished completing the evaluation click on the “Save & Close” button at the bottom of the page to save your responses.

Student Teaching Grade Form

For the purpose of the student’s seminar grade they will need to provide a screenshot of the overall grade given by their faculty supervisor for their midterm and final evaluations. Within the binder there will be a form for both evaluations in which you will need to provide an overall score. This is called the “Faculty Supervisor Grade Form”.

Once you click into the form additional instructions can be found within regarding how to score the student. NOTE: Any text space that is “red” indicates it is an area that needs to have some sort of number. The text boxes can only record a numeric value. No other characters or letters will be accepted.

Once you provide a numeric score for each text space utilize the “Save & Close” to save your responses.

NOTE: Ignore the bottom grade text box again. The red text space is the only grade input you are required to provide.

Faculty Supervisor Documentation (CA Faculty Supervisor’s Only)

Within the binder will also be a place to upload necessary evidence and documentation that is required of you as a Faculty Supervisor. The program and student teaching setting will determine if you have multiple setting spaces to provide documentation to or just one. Note: for CA Internship there will be a space for monthly uploading.

Once you have clicked on the form you will see designated uploading slots where you will submit your attendance log, faculty requirement docs, examples of communication, etc. You can either select files to upload or drag and drop files (you may upload multiple documents to each section as needed). NOTE: if a student has two placement settings, you may upload documentation to these spaces for both settings.

After you have uploaded the documents, on the bottom of the panel click “Save” if you are still needing to come back and to add more documents at a later time or “Complete” if you have successfully uploaded all documentation required.

Weekly Lesson Plan & Reflection Feedback

Lesson Plans

Throughout the student teaching you will be asked to provide feedback on the student’s weekly lesson plans that are uploaded into the binder. Located on the left-hand side of the two-panel view is a place designated for the student to upload documentation to. There is a tab titled “Lesson Plans” and you will click on this section to view the student’s reflections.

Once you click on the tab you will see any uploaded documents the student has provided. To access the uploaded document, click on the hyperlinked name of the document in a designated week to download a copy of the lesson plan the student uploaded for that week.

Weekly Reflections

Teacher candidates will complete a minimum 12-week experience. Some states or districts require candidates to complete more than the minimum required weeks. As a faculty supervisor you will be reviewing these weekly reflections as the student completes them inside the binder. To access their reflections, please locate the tab on the students binder that says “Weekly Reflections”. Once on the tab click the hyperlinked file inside the tab that says “Student Weekly Reflections”. NOTE: if the student has not begun completing reflections the file name will not be clickable.

Once you have clicked into the reflection space you will be able to see any text completed by the student under the applicable week.

Faculty Supervisor Feedback Forms.

Back under your designated forms will be a form titled “Weekly Reflection Feedback” as well as “Weekly Lesson Plan Feedback”. You will click into each form to open a series of weekly text boxes to provide corresponding feedback to what the student has uploaded on their end. You will do the same for Lesson Plans using the “Weekly Lesson Plan Feedback” form.

Note: Student will be able to view this feedback under their own binder view under a tab called “Assessment”.

CA Intern Binders

For CA Intern students, Faculty Supervisor’s will be required to complete documentation or evaluations on a monthly basis. For student binders for CA interns, all of the forms within the binder will be the same as student teaching with the exception of completion of documentation and submitting of monthly evaluations. Faculty Supervisors will see a monthly documentation form provided for them to upload a monthly copy of their informal evaluation, pencil/paper evaluations, and intern record logs.

To upload a copy of the monthly documentation, click into the monthly form to open up the documentation spaces. You may either upload a document from your computer or drag and drop the file into the space. Once your file is uploaded, please be sure to save your work at the bottom of the page!

Submitting the Binder

Throughout the binder you will see at the top of each page “Submit” and “Close” buttons. For our purposes, you will use the “Save and Close” feature to save all work at the end of the experience. You are not required to “submit” a final binder at the end of the experience.

Additional User Notes:

  • TK20 system is best used with Chrome or Firefox as your internet browser
  • Please refrain from using your browser back button while navigating inside the binder. The binder has a specific back button listed within forms to ensure that you proper navigating.
  • Do not attempt to open the system in multiple browser windows or tabs. The system only allows for a single sign window/tab. Attempts to use multiple tabs will lead to error. Should this occur exit the browser completely and repeat access efforts.
